What support will you get?

We can contact the local authority on your behalf and request an assessment of needs. The hospital and/or social services will arrange for your loved one to receive a care package bespoke to what they’re assessed as needing, including anything from physiotherapy to professional carers. The team assessing your relative will always take into account their abilities, needs and wishes including your needs, wishes and feelings.The aim is to get you and your loved one back into your daily routine and to ensure that they’re as independent, mobile, confident and medically fit as possible.Support staff will usually visit daily and work on guiding your loved one through daily tasks such as getting washed and dressed and preparing meals, helping them to rebuild confidence they may have lost while they were unwell.Health professionals and sometimes social workers will provide tailor-made exercises to help them become stronger and identify any aids and adaptations that may help them feel safer and more comfortable at home.

Care to avoid unnecessary trips to hospital (often called ‘crisis-response’)

Healthcare staff may decide that a stay in hospital isn’t necessary for your loved one. They’ll work out if their needs can be safely managed by providing short-term care at home or, if appropriate, arranging a short stay in a care home. This can avoid unnecessary hospital admission and give them the chance to recover in a more familiar environment. Statistics are showing, however that people tend to recover faster in a familiar environment such as their own home.

What happens after six weeks?

If your relative needs further support after six weeks, they will be given a plan for transferring to another service. But they may have to start paying for it themselves, depending on their circumstances.If your loved one needs long-term care or support, you can ask your local authority’s social services for a free assessment of their care needs if they haven’t already been assessed. The social worker will establish if your relative’s care package is eligible for funding from the council. If it is assessed that your relative requires long-term care, the Jordan team can continue visiting under a revised care plan.

Support for people with complex needs

If you are looking to arrange care for someone with complex care needs who is being discharged from hospital, they might also be eligible for NHS Continuing Healthcare who will fund for your loved one to be cared for at home.Jordan is accepting care packages through both the local authority and NHS with which we have long standing working relationships.
